Toyota RAV4 fuel system problems, 2016–2026
403 of the 3,510 complaints owners filed with NHTSA about the Toyota RAV4 2016–2026 name the fuel system (NHTSA’s “Fuel/Propulsion System” heading — fuel tanks, lines, pumps and injection — and on an EV, the propulsion battery and motors). The loudest years: 2019 (283), 2020 (91), 2021 (24). Filed under the same heading across these years: 1 recall campaign. Every figure is a linked federal record; none is a failure rate, none a repair cost, and none a verdict on one car.

Two things the counts carry that are not the car: how many of each year were sold, and how long each has been on the road — an older year out-counts a newer one before anything is wrong with it. The share column is the honest comparison across years: what part of that year’s complaints named the fuel system. Nothing here is a rate.
When the fuel system complaints were filed, and at what mileage

In owners’ own words

I've only filled the tank 4 times so far but I can never get it full. The most it will take is about 10 gallons and that is only if I stand there and force feed it more gas very slowly while pulling the nozzle out a little at a time. I have to fill it way too frequently, and in the winter it is especially inconvenient
My tank won't fill beyond 9-10 gallons - even when it says empty - though the fuel tank size should be 14.5. However, the tank was at max capacity when purchased from the dealer
The gas tank of my 2020 Rav4 hybrid is not filling properly. The tank is supposed to be 14.5 gallons. My fuel gauge will read just above empty and my display says that I have 29 miles to empty. When I go to fill-up my tank, though, it cuts off after roughly 8.6 - 9 gallons.
The gas tank will not completely fill at the gas station. When the gas light comes on and shows that it is empty, I will go to the gas station and it will only allow me to fill up about 9-10gallons of gas. The gas tank is listed as being 14.5gallons.
2021 RAV4 Hybrid exhibits fueling problems, consistently. Parked on level concrete, shut off, in "Refuel" status, at the fuel island, it dependably suffers abject failure at attempts to refill the gasoline tank at the fuel pump.

Common questions
Source: NHTSA complaint database, recalls and manufacturer communicationsWhich Toyota RAV4 years have the most fuel system complaints?
2019, 283; 2020, 91; 2021, 24. Raw counts, and an older year has had longer to collect them; by share of each year's own complaints the loudest is the 2019 (31%).
Is there a recall for the fuel system on the Toyota RAV4?
Yes — one campaign is filed under this heading across these model years: 20V682000 (2020). A recall is matched by VIN, and applies to specific years; check yours at nhtsa.gov/recalls.
When do fuel system problems show up on the Toyota RAV4?
Owners who gave an odometer reading reported it at a median of 1,000 miles across the model years; half of them between 0 and 5,000. That is when reports cluster, not the life of a part.
What does NHTSA's "Fuel/Propulsion System" heading cover?
Fuel tanks, lines, pumps and injection — and on an EV, the propulsion battery and motors. Owners choose the heading when they file, so it says which area of the vehicle the complaint is about, not which part failed.
