Mazda bulletin 09-005-26

What it saysIf the right-side Blind Spot Monitoring (BSM) control module is replaced for any reason, it is also necessary to replace the left-side BSM control module in order to complete the radar aiming procedure. NHTSA’s full summary ↓

Mazda filed this with NHTSA as a service bulletin dated Jan 13, 2026, received Feb 3, 2026, about the electrical system. It names 19 model years (2016–2024). A service bulletin is not a safety recall: it is the manufacturer telling its dealers how to diagnose or repair a condition, and whether the work is paid for depends on the warranty in force. Quoting the number at a service desk can save a diagnosis.

What the bulletin says

Source: NHTSA's summary of the manufacturer's communication

If the right-side Blind Spot Monitoring (BSM) control module is replaced for any reason, it is also necessary to replace the left-side BSM control module in order to complete the radar aiming procedure. If the right-side BSM control module is replaced alone, "Automatic Configuration" and "BSM Radar Aiming" cannot be completed due to a programming error in the module. In most cases, if the left-side BSM control module is replaced alone, "Automatic Configuration" and "BSM Radar Aiming" can be completed without any problem (See CAUTION below for more details). Other possible symptoms that may be found if the right-side BSM module is replaced alone: BSM aiming fails “BSM OFF" indicator ON Center Display warning message BSM failsafe (No operation) DTC U3000:54 stored in memory DTC U1A4B:54 stored in memory (CX-3, MX-5 only) NOTE: Since a new BSM control module does not retain the vehicle specification information, it is necessary to acquire and store the vehicle specification information transmitted via CAN communication from the combination meter through automatic configuration after replacement.

Read by machine from the document NHTSA serves (3 pages), read Sep 29, 2026. A part number is shown only where it matches Mazda’s own catalogue shape, an operation only with its code and time, and a description only where the reader got it whole; anything it could not settle is left out, and the PDF is the record. Times are the manufacturer’s warranty allowances, not a quote. Listings and prices on eBay Motors are the seller’s.

The summary is NHTSA’s abstract of the document, as filed under 49 CFR 579.5. One communication is usually filed as several documents — the bulletin, a dealer notice, an owner letter — and the number on any of them finds this page. A PDF link is shown only once NHTSA’s own server has been seen to serve that document; the rest are checked over the following nights. Quote the number at a service desk; it is what the dealer’s system is keyed on.

Doing the repair yourself, or pricing it before the shop does? Search the part by fitment on eBay Motors — the listing and the price are the seller’s.

Owners of the 15 model years this bulletin names have filed 191 complaints with NHTSA since 2015. 37 of them name Electrical System — the same broad component group this bulletin is filed under. NHTSA groups complaints coarsely, so that means the same area of the vehicle, not necessarily the same fault.

Since 2019 there have been several instances with my vehicle where someone has been able to knock the power offline thereby disabling it. It happened in five locations - a storage location in Olive BranchN MS, a former address in Memphis, TN (4330 Angarath Dr), a restaurant (Sonic) in Greenville, MS and twice in Greenville, MS both at a…

On the Dash the amber ABS light came on as well as the red Brake light and they stayed on for a couples and I took it to the shop and they said it was the "Steering Angle Sensor" and they changed it and the lights came back on a day later and they took another look from head to toe and they couldn't find a reason why the ABS and Brake…

A complaint is one owner’s account, not a defect finding, and a bulletin is not a recall. Nothing here says this bulletin was issued because of these complaints, or that the repair it describes fixed them. Complaint files on this site begin in 2015.

Common questions

Source: NHTSA, 49 CFR 579.5

Is Mazda 09-005-26 a recall?

No. A service bulletin is a manufacturer communication filed with NHTSA under 49 CFR 579.5; a safety recall carries an NHTSA campaign number (two digits, a letter, six digits) and a federal duty to repair for free. Check the vehicle’s recalls by VIN at nhtsa.gov/recalls, or on its page here.

Is the 09-005-26 repair free?

Not automatically. A service bulletin describes how to diagnose or repair a condition; who pays depends on the warranty in force, any extension, and the dealer. Quoting the number can save a diagnosis either way.

Which vehicles does 09-005-26 cover?

The manufacturer filed it against 19 model years: 2021 Mazda CX-3, 2020 Mazda CX-3, 2019 Mazda CX-3, 2018 Mazda CX-3, 2017 Mazda CX-3, 2016 Mazda CX-3 and more. Whether one particular vehicle is affected is a matter of its VIN and its build, not its model year alone.
