Mazda bulletin 09-011-26

What it saysThis bulletin supersedes the previously issued bulletin(s) listed below. NHTSA’s full summary ↓

Mazda filed this with NHTSA as a service bulletin dated Feb 26, 2026, received Mar 3, 2026, about the exterior lighting. It names 5 model years (2024–2025). A service bulletin is not a safety recall: it is the manufacturer telling its dealers how to diagnose or repair a condition, and whether the work is paid for depends on the warranty in force. Quoting the number at a service desk can save a diagnosis.

What the bulletin says

Source: NHTSA's summary of the manufacturer's communication
Built before 11 Apr 2025 “Produced before April 11, 2025”
Built before 27 Sep 2025 “Produced before September 27, 2025”

What it applies to, as the document itself states it — the build window in the manufacturer’s own words, read by machine from the PDF NHTSA serves. The build date is on the certification label in the driver’s door jamb; the dealer settles a borderline vehicle by its VIN.

This bulletin supersedes the previously issued bulletin(s) listed below. The changes are noted in Red. 09-019/25 04/15/25 Some vehicles equipped with the Adaptive Front lighting System (AFS) may exhibit the exterior light warning light/exterior light warning indication on while driving or when the ignition is turned on and DTC B10A3:86 (Error signal received from swivel actuator (LH)) stored in memory. NOTE: The combination light with AFS can be distinguished if it is equipped with the parking light/daytime running light (DRL) that illuminates while the ignition is turned on. This may be caused by the AFS Control Module detecting a false headlight malfunction. This concern will likely occur when the light returns to the center position after swiveling. To eliminate this concern, the control software of the AFS control module has been further modified.

Read by machine from the document NHTSA serves (3 pages), read Sep 29, 2026. A part number is shown only where it matches Mazda’s own catalogue shape, an operation only with its code and time, and a description only where the reader got it whole; anything it could not settle is left out, and the PDF is the record. Times are the manufacturer’s warranty allowances, not a quote. Listings and prices on eBay Motors are the seller’s.

Affiliate disclosure: some links on this page are affiliate links. If you request a quote or buy through them we may earn a commission at no extra cost to you. Nothing on this page changes because of it.

The summary is NHTSA’s abstract of the document, as filed under 49 CFR 579.5. One communication is usually filed as several documents — the bulletin, a dealer notice, an owner letter — and the number on any of them finds this page. A PDF link is shown only once NHTSA’s own server has been seen to serve that document; the rest are checked over the following nights. Quote the number at a service desk; it is what the dealer’s system is keyed on.

What owners reported

Source: NHTSA complaint database

Owners of the 5 model years this bulletin names have filed 542 complaints with NHTSA since 2015. 2 of them name Exterior Lighting — the same broad component group this bulletin is filed under. NHTSA groups complaints coarsely, so that means the same area of the vehicle, not necessarily the same fault.

The vehicle has a recurrent malfunction of the Adaptive Front-lighting System (AFS), part of the exterior headlight system and advertised as a safety feature within Mazda’s i-ACTIVSENSE® technology. The failure presents as a dashboard warning stating: “Headlight System Malfunction.

2024 Mazda MX-5 · Exterior Lighting · filed with NHTSA Dec 20, 2025

After driving on slight up or down hill with headlights on, I'm getting Headlight malfunction warning on my almost brand new (700 miles) Miata

2025 Mazda MX-5 · Exterior Lighting · filed with NHTSA Jul 20, 2026

A complaint is one owner’s account, not a defect finding, and a bulletin is not a recall. Nothing here says this bulletin was issued because of these complaints, or that the repair it describes fixed them. Complaint files on this site begin in 2015.

Common questions

Source: NHTSA, 49 CFR 579.5

Is Mazda 09-011-26 a recall?

No. A service bulletin is a manufacturer communication filed with NHTSA under 49 CFR 579.5; a safety recall carries an NHTSA campaign number (two digits, a letter, six digits) and a federal duty to repair for free. Check the vehicle’s recalls by VIN at nhtsa.gov/recalls, or on its page here.

Is the 09-011-26 repair free?

Not automatically. A service bulletin describes how to diagnose or repair a condition; who pays depends on the warranty in force, any extension, and the dealer. Quoting the number can save a diagnosis either way.

Which vehicles does 09-011-26 cover?

The manufacturer filed it against 5 model years: 2025 Mazda CX-70, 2025 Mazda CX-90, 2024 Mazda CX-90, 2025 Mazda MX-5, 2024 Mazda MX-5. Whether one particular vehicle is affected is a matter of its VIN and its build, not its model year alone. The document itself sets the build window: built before 11 Apr 2025; built before 27 Sep 2025 — the build date is on the certification label in the driver’s door jamb.
